Veteran's Administration (VA) Medical Services |

Employer-Provided Health Insurance |
Health insurance coverage offered through one's own employment or a relatives. It may be offered by an employer or by a union. This includes TRICARE which is available to veterans based on military service. |

Health insurance obtained through COBRA |
The Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA) requires most employers with group health plans to offer employees the opportunity to continue temporarily their group health care coverage under their employer’s plan if their coverage otherwise would cease due to termination, layoff, or other change in employment status (referred to as “qualifying events”). |

Private Pay Health Insurance |
Health insurance coverage purchased by an individual from a private health insurance company. |

State Health Insurance for Adults |
ConnectorCare is a program for Massachusetts residents with incomes of three times the Federal Poverty Level or less (about $71,550 or less for a family of four) who can qualify for help from the state and federal government to pay for their health insurance. ConnectorCare provides similar benefits to Commonwealth Care. |

Indian Health Services Program |
The Indian Health Service (IHS), an agency within the Department of Health and Human Services, is responsible for providing federal health services to American Indians and Alaska Natives. |

One Care: MassHealth plus Medicare: Clients who are enrolled in One Care should answer “Yes” to both having Medicaid and Medicare, as this program is an integration of both for those who are eligible.
Insurance access through the federal or a state Health Exchange (e.g. healthcare.gov), except for Medicaid should be recorded as Private Pay Health Insurance, even if a subsidy covering most of the cost of insurance was provided.
