Exit Destination
The Exit Destination describes where the client will be staying just after exiting the project. This data is collected for all clients, including children, so that instances where household members exited at different times or to different destinations can be accurately reflected.
If a client exits without providing destination information to project staff, the “No exit interview completed” response value should be used; in such instances, destination information will be considered missing. If the exit destination is learned later staff may edit the Exit to reflect the correct destination.

The client has exited to a hospital for any reason other than psychiatric. |

The client has been arrested and is residing in a local jail, prison (state or federal) or juvenile detention facility. |

Includes any residential care involving a medical need (for example, a nursing home or rehabilitation center). |

This could be a scattered-site or site-based supportive housing where the rental subsidy is from CoC Permanent Supportive Housing, HOPWA facility/TBRA permanent housing (when moving from non-HOPWA projects), or a local source of subsidy restricted strictly for homeless persons. |

Place Not Meant for Habitation |
The client has exited to sleeping on the street or any place not meant for human habitation (for example, a vehicle, abandoned building, storage unit, parking garage, bus/train/subway/airport station, campsite, or anywhere outside). |

Psychiatric hospital or other psychiatric facility |
The client has exited to a psychiatric facility or psychiatric hospital, or psychiatric unit of a local hospital. |
|
Rental by client, no ongoing housing subsidy |
The unit the client is renting is not supported by any government or private subsidy. |
|
Rental by client, with GPD TIP housing subsidy |
The unit the client is renting is being supported by a Grant Per Diem Transition-in-Place subsidy. This is a Veterans Affairs (VA) funded program. |

Rental by client, with other ongoing housing subsidy |
The unit the client is renting is being supported by a subsidy – either government or private, either site-based or voucher other than CoC PSH, HOPWA PH, RRH, GPD TIP, or VASH. Includes HUD HCV with no paired services, legacy SRO, and Pay for Success. |

Rental by client, with RRH or equivalent subsidy |
Use this response category only if the client is moving directly into a unit. This includes CoC Rapid Re-Housing, ESG Rapid Re-Housing, SSVF Rapid Re-Housing, VA GPD Transition in Place, or other locally-funded Rapid Re-Housing projects. |

Rental by client, with VASH subsidy |
The unit the client is renting is being supported by a HUD/VASH (Veterans Affairs Supportive Housing) subsidy. |

Residential project or halfway house with no homeless criteria |
The client exited to a residential project or halfway house that has no homeless requirement, no lease or rights of tenancy, with or without time limits. |
|
Safe Haven |
Program for those who are unwilling or unable to participate in supportive services. Must be formally designated as a Safe Haven. |

Staying or Living with Family, Permanent Tenure |
The client has moved into a room, apartment or house occupied by a family member and is intending on living there. Use “permanent” if the client has NOT been given a specific time limit in which he/she needs to leave or if exit destination is short term but leads to a permanent destination such as doubled up for two weeks until an apartment or other permanent living situation is ready. |

Staying or Living with Family, Temporary Tenure (e.g., room, apartment or house) |
The client has exited to a room, apartment or house occupied by a family member and is intending on staying there only a short time. Use “temporary” if client is given a time limit in which he/she needs to leave or if the Case Manager has knowledge that the destination is meant to be very short term and/or is lacking stability, such as overcrowding (more than 1.5 persons per room). |

Staying or Living with Friends, Permanent Tenure |
The client has moved into a room, apartment or house occupied by a friend and is intending on living there. Use “permanent” if the client has NOT been given a specific time limit in which he/she needs to leave or if exit destination is short term but leads to a permanent destination such as doubled up for two weeks until an apartment or other permanent living situation is ready. |

Does NOT include an exit to substance abuse treatment facility. |
